Alternatively, for travel where departure date is a minimum of 12 weeks in the future you can complete and return a self-assessment risk form for every person travelling to the practice by handing them in at reception or submitting them online through accuRx https://florey.accurx.com/p/C82021
FORMS WILL NOT BE ACCEPTED AFTER THIS TIME SCALE
A telephone consultation will be booked with the Practice Nurse within 14 days after receipt of the form(s) to discuss the travel risks and a further appointment will be booked if required. Please note that not all vaccinations can be administered at the Surgery and you may be directed to a travel clinic.
For departure dates less than 12 weeks in the future and last minute travel, please visit a travel clinic.
